PO (Adults): 20 mg once daily with evening meal. May be ↑ at 4-wk intervals to a maximum of 80 mg/day (immediate-release) or 60 mg/day (extended-release); initiate at 10 mg/day in patients receiving cyclosporine or other immunosuppressants and do not exceed 20 mg/day; should not exceed 40 mg/day (immediate-release) or 20 mg/day (extended-release) if receiving verapamil or amiodarone; should not exceed 20 mg/day if receiving danazol or niacin (>1 g/day)..Renal Impairment
