The prevention or hindering of the coagulation of blood, esp. by an anticoagulant drug. Algorithms are often used to optimize warfarin anticoagulation, e.g., warfarindosing.org. Direct oral anticoagulants, like apixaban and rivaroxaban are dosed based on body weight and indication and do not require algorithmic dosage adjustments.
