This webinar, featuring authors of a Clinical Simulation in Nursing article, detailed a collaborative project on using generative AI to create nursing simulation scenarios. The project goal was to streamline the scenario design process while upholding rigorous quality standards.

Their methodology involved a team-based approach to test various AI platforms with diverse clinical cases. They used the INACSL standards and the NLN Simulation Design Template as a framework for their prompts. A key takeaway was the importance of structured "prompt engineering"—crafting clear, systematic instructions for the AI. The team found that using frameworks like CLEAR and reordering the prompts from the standard NLN template yielded more consistent and useful outputs.

The webinar’s central theme is that AI is a powerful assistant that augments, not replaces, the simulationist’s expertise. The presenters discovered significant issues requiring expert intervention: inherent biases and stereotypes in patient profiles, which often generated identical and stereotypical demographics for specific diagnoses across different platforms. The outputs also contained clinical inaccuracies or "hallucinations," with irrelevant cognitive and psychomotor skills suggested for pre-briefing. Furthermore, the AI created over-ambitious scenarios with unrealistic timelines that compressed activities that would take students 20-30 minutes into an impossible 5-minute interval.

Educators must serve as the final arbiters of quality, using their expertise to fact-check, correct bias, ensure clinical accuracy, and align scenarios with learning objectives. Ultimately, GenAI is a valuable tool, but its responsible and effective use depends entirely on expert human guidance.

Dr. Liz Robison, a Certified Healthcare Simulation Educator-Advanced and Certified Nurse Educator, is an independent consultant with extensive experience in simulation and patient safety, having served 22 years in the US Air Force Nurse Corps and nearly 16 years in prelicensure nursing education. Currently co-chair of OADN’s Simulation Committee, she has presented on using generative AI in nursing curriculum and worked internationally on incorporating AI into simulation scenarios.

Theresa Cooney, MSN, RN

Theresa Cooney is an experienced nurse with 35 years of international experience, specializing in pediatrics, and has transitioned to academia with a focus on nursing education and simulation. At Howard Community College, she teaches a concept-based curriculum, contributes to curriculum development and faculty professional development, and is actively involved with OADN’s Virtual Simulation Review team, demonstrating her commitment to advancing nursing education through innovative teaching methods and technology integration.

Dr. Tammy Schwaab, Simulation Director and Professor at Carroll Community College, is a Certified Healthcare Simulation Educator with experience in simulation since 2010 and nursing education since 2001. Her DNP project explored using clinical simulation to improve new graduate nurse confidence. She actively contributes to the field through leadership and membership in organizations like OADN, INACSL, MCSRC, and the Maryland Community and College Simulation User Network, where she served as Secretary and President. She’s also a member of the Society for Simulation in Healthcare.
